Our Personal Support Service offers a range of home and community-based services intended to assist people to participate fully in their home and community life. Personal support services are provided based upon the needs and choices as articulated in the Person-Centered Plan.
Personal Supports may include but is not limited to assistance, supervision, and training in the following:
• Financial Management
• Locating Appropriate housing •Grocery Shopping
• Medication Administration
• Medical Care Coordination
• Light Housekeeping
• Self-Advocacy
• Community Resources
• Health and Safety Plan
• Interpersonal Skills
• Adaptive Technology
• Mobility
• Pursuit of specific individual goals
Our Supported Living Services module provides a person-centered approach to services that consider the uniqueness and abilities of each person served, regardless of disability. It was established to support those who wish to live more independently and is customized to meet each individual’s needs. It may include support for just a few hours a week or daily “drop-in” by staff for those individuals who are relatively independent. Individuals reside alone in their apartment or may choose to live with a roommate. A roommate can be another participant receiving DDA services from either COIPSS or another DDA provider; or another person he or she chooses, but does not include relatives, legal guardians, or legally responsible persons.
COIPSS will provide supported living services to more than one participant under this provision. Supports provided vary according to individual needs as indicated in the PCP and Nursing Assessment when applicable and may include assistance with:

• Medical Care Coordination
• Light Housekeeping
• Self-Advocacy

Respite is an “indirect” service that provides relief to people who are responsible for the primary care and support of a person with a developmental disability. Respite Services are the provision of short-term, temporary relief to those who are caring for family members enrolled in the Waiver. Respite care will ensure that persons will continue to receive services and have access to community activities as described in their PCP including transportation to and from the activities. Families have the option of using a COIPSS support staff or someone they choose to provide respite support. Family-Chosen caregivers will be required to abide by the same onboarding process as COIPSS support staff. Respite Service can be planned or in emergent situations. Supports provided vary according to individual needs as indicated in the PCP and Nursing Assessment when applicable, and may include assistance with:

· Meal Preparation
· Medication Administration
· Light housekeeping
· Self-Advocacy
· Community Resources
· Health and Safety
· Interpersonal Skills
